Telemetrix, Inc. (“TELEMETRIX” or “We”) is committed to protecting your privacy and providing Services in a manner that provides a powerful and safe user experience. During the course of providing Services, TELEMETRIX may collect personally identifiable information such as name, address, e-mail address as well as Protected Health Information (“PHI”). This information is used by TELEMETRIX for the operation of the Services, to maintain quality of the Services and to provide general statistics necessary to administer our Services.
To the extent that TELEMETRIX collects and stores PHI, it is required by law to make a reasonable effort to maintain the privacy of PHI to which we come in contact. TELEMETRIX is further required to provide you with access to this notice of our legal duties and our privacy practices regarding your PHI.
Below is a description of how TELEMETRIX may use and disclose information about you and your Services (including but not limited to PHI). Except for the purposes described below, we will use and disclose your personal and private information only with your written permission.
THIS NOTICE DESCRIBES HOW INFORMATION ABOUT YOU (INCLUDING BUT NOT LIMITED TO PROTECTED HEALTH INFORMATION) MAY BE USED AND DISCLOSED BY TELEMETRIX, INC. IT ALSO DESCRIBES HOW YOU CAN GET ACCESS TO THIS INFORMATION. TELEMETRIX IS REQUIRED TO ABIDE BY THE TERMS CONTAINED WITHIN THIS NOTICE OF PRIVACY PRACTICES, AS MAY BE MODIFIED FROM TIME-TO-TIME. BY USING THE TELEMETRIX, INC. SERVICES, YOU CONSENT TO THE PRIVACY PRACTICES DESCRIBED IN THIS STATEMENT. PLEASE REVIEW IT CAREFULLY.
In regard to Services provided in association with health care professionals, TELEMETRIX may distribute PHI and other information to health care providers with which you have an established relationship, including, for example, to a patient's attending physicians. We may also provide access to PHI and other information in the course of submission of claims to Medicare, Medicaid, insurance companies, and other organizations responsible for payment of Services. In the above instances of disclosure, we may share information with supervisors of staff members providing care to patients.
In the course of providing Services, TELEMETRIX may also utilize personal information including PHI to initiate contact with the patient, patient representative, care provider, or medical provider. Such contact may be made to alert named parties of specific medical conditions, to schedule visits, and/or for other coordination of care activities. You understand and accept that such contacts may be made in the normal course of providing Services and may be made through electronic means.
Furthermore, TELEMETRIX may use or disclose PHI and other personal information without an individual’s written consent or authorization where authorized or required under law. Such releases may be made with regard to (but are not limited to):
- Disclosure to patients, incident to another permitted use or disclosure
- The Secretary of the U.S. Department of Health and Human Services
- Coroners, medical examiners, and funeral directors
- Lawsuits and disputes pursuant to court order
- Data breach notification purposes
- Health oversight activities
- Public health risks
- Worker’s compensation claims
- Military and veterans government organizations
- Organ and tissue donation centers
- Disaster relief providers
- The U.S. Food and Drug Administration
- As otherwise may be required by federal, state, or local law
Access to personal information including PHI may also be released on a need-to-know limited basis to care providers in instances where you or your personal representatives are unable to provide permission and where such release may avert or minimize a serious threat to your health or safety, including abuse, neglect, or domestic violence.
We will retain any personal information or PHI only as necessary for the provision of our Services and as required by law.
We may collect, utilize, and share aggregate data including de-identified personal information including PHI in the course of providing, evaluating, and updating our Services. Use of aggregate de-identified data will follow all relevant law and regulation and best case business practices.
TELEMETRIX sites may use "cookies" or similar technology to help you personalize your online experience. A cookie is a text file that is placed on your hard disk by a Web page server. One of the primary purposes of cookies is to provide a convenience feature to save you time. The purpose of a cookie is to tell the Web server that you have returned to a specific page. For example, if you register with a TELEMETRIX site or Services, a cookie helps Us to recall your specific information on subsequent visits. This simplifies the process of recording your personal information, such as billing addresses, shipping addresses, and so on. When you return to the same TELEMETRIX site, the information you previously provided can be retrieved, so you can easily use the TELEMETRIX features that you customized.

TELEMETRIX follows generally accepted industry standard security practices to protect your personal information from unauthorized access, use, or disclosure. We secure the personally identifiable information, including but not limited to PHI, that you provide on computer servers in a commercially reasonable, controlled, secure environment, protected from unauthorized access, use, or disclosure. Where appropriate and necessary for legal compliance, data in association with our Services is stored and transmitted in encrypted form.
TELEMETRIX will not sell, rent, or lease access to any subscriber information to any unaffiliated party not involved in providing Services to you. Furthermore, TELEMETRIX does not use or disclose sensitive personal information which it may obtain in the course of providing Services, such as race or religion, without your explicit consent.
Sharing of any PHI with affiliated entities and vendors providing underlying services to TELEMETRIX will be in full accordance with requirements associated with Business Associate Agreements which will impose restrictions upon that third party's use of private data that are at least as restrictive as that outlined in this Notice of Privacy Practices. All such third parties are prohibited from using your personal information except to provide services to TELEMETRIX.
Our Service may contain links to other websites that are not operated by Us. If You click on a third-party link, You will be directed to that third party's site. We strongly advise You to review the Privacy Policy of every site You visit.
We may from time-to-time contact you on behalf of associated entities about a particular offering that may be of interest to you. In those cases, your unique personally identifiable information will not be transferred to any third party.
TELEMETRIX will notify affected individuals and all other parties required by law in case of breach of unsecured PHI.
The individual has rights regarding PHI and its use. You have the right to request further restrictions on certain uses and disclosures of PHI. However, TELEMETRIX is not required to agree to any requested restrictions.
You have the right to inspect and copy PHI collected and maintained by TELEMETRIX, the right to an electronic or paper copy of PHI, the right to be notified in the event of a breach relative to PHI and other private information collected and maintained by TELEMETRIX, the right to amend PHI which TELEMETRIX may maintain, and the right to an accounting of disclosures of PHI by us.
Relative to personal information collected and maintained by TELEMETRIX in the course of providing our Services, you have the right to request confidential communications with us. You also have the right to receive a paper copy of this notice upon request. If you have agreed to receive this notice electronically, you may obtain a copy at our web site https://privacy.telemetrix.io.
Relative to messaging and outreach, you agree to receive text messages from us when order consent is received. Telemetrix will send SMS messages to communicate with customers on the status of the projects they have requested to provide a better overall customer experience. For HELP, please contact us via text, phone, or email. To opt-out of receiving messages, reply with “STOP” at any time. Message frequency may vary. Standard message and data rates may apply. No mobile opt-in will be shared with third parties for marketing purposes.
TELEMETRIX reserves the right to change this notice and to make the new notice effective for all PHI and other private/personal information maintained in the course of providing our Services.
If you feel as though your privacy rights have been violated relative to PHI, individuals may complain to the organization and to the Secretary of the U.S. Department of Health and Human Services; the Toll-free Hotline number is 1.800.227.7308.
